A leading mental health charity in the South West seeks a dedicated team member to support clients facing complex needs, including mental health and homelessness. You will engage with clients using a strengths-based approach, providing guidance through assessments and support planning in a collaborative recovery-focused environment.
Step Together is a trailblazing service that offers a psychological, adversity and trauma (PAT) informed approach to those rough sleeping/at risk of homelessness with multiple, complex needs (mental health, forensic history, substance use) in Somerset. We advocate for system change, creatively thinking with other agencies to develop innovative ways to overcome barriers to engagement, turning these into opportunities to steer positive trauma-informed system change.
Working to the Recovery Coach Model principles, you will develop strengths-based actions orientated around the Outcomes Star. You will engage and support your own caseload of clients with complex needs in leading their journeys, and maximising their choice and control over the support they receive by developing a responsible and individual network of support.You will have an understanding of issues relating to homelessness and mental health, and be able to build and sustain relationships with clients. You will have a commitment to promoting independence and supporting vulnerable people in developing move on and recovery plans.We work in a recovery focused, psychological, adversity and trauma (PAT) informed way in all aspects of the teams practice using relevant interventions, including Assessments, Support Planning, Risk Assessing, Brief Interventions and Solution Focused Therapy.

We work with people who experience complex needs associated with multiple and severe disadvantage (including psychological distress, homelessness, substance misuse and contact with the criminal justice system).
Conduct and support clients assessmentsPositively manage and reduce clients using safety and inclusion planning toolsWorking with a caseload, undertake proactive work across Somerset to engage and build trusting relationships with clients. Developing a full understanding of their backgrounds, current situation, their priorities, needs, hopes and ambitions.Provide a single consistent point of contact for a client and other people working with them.Develop strength-based actions, orientated around the outcomes Star. Support each client in leading their journey at a pace that is right for them, and review progress on actions with them on a regular basis.
