A UK-based mental health service provider is seeking a dedicated professional to provide support to users with substance misuse challenges. Responsibilities include conducting assessments, implementing recovery plans, and collaborating with a diverse range of professionals to enhance user experiences. To work as a key member of a multi-disciplinary team by offering information, advocacy and support to service users with substance misuse, in line with their recovery action plan, to improve user experience and promote recovery.
To undertake needs-led assessments of people with substance misuse problems requiring additional support to address social needs. To inform recovery action plans in consultation with service users and their case workers, addressing social care needs. To implement, monitor and review a range of interventions related to social needs. To support and motivate service users to attend appointments and activities promoting recovery. To advocate for service users where appropriate, to work in partnership with other professionals and agencies, and to engage with mutual aid/self-help groups to support recovery and independence. To involve families and carers where appropriate and to encourage safer, healthier lifestyles. To promote service user and carer involvement in planning and delivering services and to participate in team meetings.

The Trust mandates DBS Update Service for roles requiring a DBS; enrolment is required for a fee of £16 per year and ongoing registration is expected during employment with GMMH.
