Job description

RECONNECT Team Leader

Location: Hybrid working Cambridgeshire

Salary: £27,992 per annum plus benefits

Vacancy Type: Permanent, Full Time

We are currently recruiting for a RECONNECT Team Leader to cover the Cambridgeshire area, including hybrid working at HMP Whitemoor, HMP Peterborough (male & female), HMP Woodhill, and HMP Littlehey.

We are Forward, a social enterprise empowering people to break the cycle of crime and addiction, helping them move forward to live fulfilling lives.

RECONNECT is a care after custody service aimed at improving continuity of care for individuals leaving prison or immigration removal centres (IRC) with identified health needs.

About the role:

You will oversee a comprehensive, inclusive, and bespoke RECONNECT service, providing advice and guidance to clients across HMP Whitemoor, HMP Peterborough (male & female), HMP Woodhill, HMP Littlehey, and community services. You may also assist with other establishments and regions, remotely or in person.

The service supports service users in accessing and maintaining interventions related to physical and mental health, and overall well-being post-release. It includes signposting, GP referrals, appointment accompaniment, meet and greet at the gate, aftercare support, and connecting clients to the wider Forward community. Following a referral 12 weeks prior to release, you will assess clients' needs and collaborate with various agencies to develop personalized care plans.

Key responsibilities:

As a RECONNECT worker, you will contribute to service targets set by Forward and local commissioners, assess and respond to client needs, and communicate with internal and external agencies to ensure high-quality service delivery. You will actively seek client feedback to improve practices.

Some key responsibilities include:

  1. Implement a comprehensive, robust strategy aligned with the RECONNECT contract.
  2. Build and maintain partnerships with external agencies and stakeholders.
  3. Recruit and develop volunteers and peer support workers with lived experience.
  4. Manage a team, including supervision, reviews, and team meetings.
  5. Establish and maintain referral pathways for stakeholders, including custodial partners, families, and self-referrals.
  6. Lead by example in line with Forward Trust values and mission.
  7. Deliver a high-quality, safe service through collaboration, risk assessments, and safeguarding.
  8. Ensure teams are connected with local services for client release.
  9. Support recruitment, training, and retention of staff and volunteers.
  10. Collect data, report KPIs, and respond to information requests.
  11. Assist your line manager as needed.

All prison-based roles require an enhanced DBS and HMPPS security vetting, which can take 8-12 weeks. Offers are subject to vetting clearance.

Checks include:

  • Personal, financial, police, and criminal history information.
  • Family details.
  • Co-residents and associations that may conflict with the role.
  • Background checks across police systems, credit references, social media, and other agencies.

The ideal candidate will have:

  • Strong communication skills and the ability to represent Forward Trust professionally.
  • A diploma in Health & Social Care or relevant field (desirable).
  • Line management experience and team leadership skills (desirable).
  • Experience facilitating groups, knowledge of motivational interviewing, and criminal justice sector experience (beneficial).
  • Good organizational skills and experience with databases (desirable).
