Forward Trust is seeking a Peer Support Worker for the RECONNECT service to support people leaving prison in Hertfordshire and beyond. The role combines custody and community work, delivering person-centred care and coordinating with health and social care services.
You will assess, co-create care plans, and provide up to six months post-release support, contributing to health equity and successful reintegration with a compassionate, collaborative approach.
