Oxleas NHS Foundation Trust is recruiting for a CYP Integrated Neighbourhood Team Project Manager for Bexley. The role is a fixed term or secondment for 1 year, with a base at Memorial Hospital and flexible site work including some home-based work.

You will support the development and implementation of Bexley’s Integrated Neighbourhood Teams for Children and Young People, coordinating with ICB and partners to drive outcomes and reduce health inequalities.

Qualifications

  • Experience leading change and coordinating projects across multiple partners.
  • Ability to work with ICB, community, primary, acute health services, education, social care and voluntary sector.
  • Experience in CYP services is desirable.

Responsibilities

  • Lead the development and implementation of Bexley’s CYP Integrated Neighbourhood Team model.
  • Work with partners to co-produce and implement the CYP INT plan.
  • Attend meetings across the system and escalate risks or concerns as needed.
  • Lead on data collection and evaluation to inform planning and improvements.

The Children and Young People Integrated Neighbourhood Team Project Manager for Bexley is an exciting new role that offers the right candidate a fixed term or secondment opportunity for 1 year.

Working closely with ICB, community, primary, acute, education and social care colleagues you will support the development and implementation of Bexley's Integrated Neighbourhood Teams (INTs) for Children and Young People (CYP).

This role would suit someone with experience of leading change and coordinating projects, who can work with a number of different stakeholders to take this plan forward and help improve outcomes for children and young people in Bexley.

Although your base will be Memorial Hospital the post holder will work across a number of different sites flexibly, as well as some time where working from home would be an available option.

Nationally, ICB areas are tasked with implementing INTs in 2026/27 to priority cohorts. The post holder will work alongside partners in the ICB, community, primary, acute health services as well as education, social care and the voluntary sector to co-produce and implement Bexley’s CYP INT model. Your work will help create joined-up, person-centred services that tackle health inequalities and genuinely improve outcomes for local people– as set out in the NHS 10-year plan.

Translate national policy and strategy into practical local delivery

Enjoy bringing structure to ambiguity and helping teams move from ideas to action

Ensure that reducing inequalities and improving outcomes for communities is embedded throughout planning, delivery and evaluation.

Attend meetings across the system

Acting as the key lead to escalate risks or concerns

Leading on data collection and evaluation

Organising or commissioning training as needed

Setting up, maintaining and developing project documentation

Work with partners to coordinate budgets and the best utilisation of investment, ensuring there are no duplication of resources

Collaborate with the Leadership Team to explore ideas and develop business cases for future developments within the Bexley CYP INT programme, and what they mean in practice for people, time and resources

Look ahead at activity and demand to help plan and shape future initiatives

Monitor how services are running, spotting risks or issues early and working with partners to address them

Support clear decision-making and accountability, helping neighbourhood work stay focused and well-governed

Help teams across primary care, social care, community services and hospitals work as one around people’s needs

To undertake some routine administrative tasks as required by the role e.g. Assisting with the organisation of meetings and events
