City Health Care Partnership CIC in Willerby is seeking a Case Manager to provide expert care coordination for patients with complex health needs and long-term conditions within a community nursing team.
You will assess, plan, deliver and review care, collaborating with patients, carers and partner organisations to prevent hospital admissions and improve outcomes. This role emphasizes multidisciplinary teamwork, supervision, and continuous professional development.
Working with a community nursing team, you’ll provide expert care and coordination for people with complex health needs, helping them stay well and receive the right support at the right time. Based within a multidisciplinary service, you’ll play a key role in delivering high-quality care for patients with long-term conditions while helping to prevent unnecessary hospital admissions.
You’ll assess, plan, deliver and review care, working closely with patients, carers and partner organisations. You’ll also support colleagues, contribute to service improvement and help maintain high standards of clinical practice across the team.
