Job summary

HTG-UK is seeking a Patient Transport Liaison Officer to join the Transport Desk at Norfolk and Norwich University Hospital on a temporary contract up to 6 months, with potential for extension. The role is on-site and 37.5 hours per week.

You will coordinate patient discharges, liaise with ward staff and outpatient clinics, and ensure timely transport while meeting KPI targets across the region.

Exciting full-time opportunity available for a Patient Transport Liaison Officer to join our team at the Transport Desk, Norfolk and Norwich University Hospital on a temporary basis for up to 6 months, with the potential for the role to become permanent following the initial contract period

What Can You Expect?

We are looking for a dedicated individual to join us in the role of Patient Transport Liaison Officer on an initial temporary contract for up to 6 months, with the potential to become permanent. Working 37.5 hours per week, you will play a vital role in supporting safe and timely patient discharges while ensuring effective transport coordination across hospitals throughout the region.

In this role, you can make a huge difference to patients' journeys through liaising with healthcare professionals at all levels, both internally and externally. You will help ensure effective patient flow within the Norfolk and Norwich Hospital, supporting contractual requirements and aligning with wider healthcare system priorities.

The role responsibilities will include coordinating patient discharge journeys and, where possible, integrating these with existing planned outpatient transport to maximise efficiency and improve the patient experience. This will involve negotiating collection times with ward and site teams, as well as working closely with outpatient clinics to effectively plan patient arrivals and departures, including coordinating and adjusting appointment times where required to ensure a smooth and seamless transport journey.

Summary Of Main Duties Include:
  • Be based on-site at NNUH desk.
  • Support the control and planning team to ensure efficient patient flow between the Acute Hospital and Patient Transport Provider, with a focus on discharges.
  • Prioritise patient transport needs within the service to maximise the efficient use of transport resources.
  • Collaborate closely with hospital site teams and control teams to ensure smooth and effective patient flow.
  • Attend daily bed meetings and work with Clinicians and Bed Managers to align priorities and assist with time-critical discharge journeys.
  • Ensure discharges from your acute site are planned within KPI targets.
  • Assign patients to the correct transport crew/resource and verify patient locations.
  • Accurately update confidential patient information in hospital and patient transport IT systems.
  • Work with ward staff to ensure patients are ready for transport, and mobilities are correct. This will include going to wards to assess mobilities.
  • Escalate potential issues to the Customer Relations Manager (CRM) promptly.
