Candlelight Care is seeking dedicated individuals for its expanding D2A Team in the Mendip District and Somerset. This role involves providing crucial support and care to patients, helping them transition smoothly from hospital to home. Our DISCHARGE TO ASSESS (D2A) Team is expanding. This is a rare opportunity being offered!

We provide a short-term care and reablement service in patients' own homes, to bridge the gap between hospital and home.

This means that patients no longer need to wait unnecessarily for assessments in hospital.

In turn, this reduces delayed discharges and improves patient flow.

Grade 5: £12.60/hr, paid travel time and mileage, uniform, Blue Light Discount Card.

Location: Mendip/Somerset areas

Previous care experience is desirable but not essential.
