Job summary

Nexxus Care is seeking empathetic Reablement Support Workers to help people stay independent at home. You will support clients to regain independence, develop daily living skills and build confidence in everyday activities, working with therapists and senior staff to implement personalised plans.

Role involves visiting clients in the community across Staffordshire, driving between visits, and ensuring safe discharge from hospital when needed.

Qualifications

• Must hold a full UK driving licence and have access to your own vehicle.
• Ability to travel across the Staffordshire area to visit clients.
• Demonstrates care, empathy and a supportive approach to clients.
• Good communication and interpersonal skills are essential.
• Ability to work independently as well as part of a team.

Responsibilities

• Support clients in their own homes to regain independence and develop daily living skills.
• Follow personalised care and reablement plans with therapists and senior staff.
• Provide compassionate, person-centred care and monitor progress.
• Maintain accurate records and documentation of visits.

About The Role

About the Role

At Nexxus Care, our Reablement Support Workers play an important role in helping people remain independent in their own homes for as long as possible.

You will support clients to regain their independence, develop new skills and build confidence with everyday activities. Working as part of a wider care team, you will work closely with Occupational Therapists and Senior Support Workers to provide personalised, practical support.

The role also helps to support safe hospital discharge, enabling clients to return home and regain their independence rather than requiring ongoing hospital or care support.

Key Responsibilities

• Support clients in their own homes to regain independence
• Encourage and develop clients' daily living skills
• Follow personalised reablement and support plans
• Work alongside Occupational Therapists and Senior Support Workers
• Support clients with achieving their individual goals
• Provide compassionate, person-centred care
• Travel between clients within the Staffordshire community
• Maintain professional standards and accurate records

About You

You will need:
