Overview

Reablement Support Lead (Days) 8981 | Permanent contract | £25,753 to £27,936 per annum (Scale F) | 33.32 hours per week | Grays Fair Court, Norwich

Weekends and bank holidays on a rota basis. Working 1 in 3 weekends.

Would you like to work in a Housing with Care setting, supporting people to live independently with dignity and choice? Then this role could be perfect for you. Grays Fair Court is part of the wider Norfolk First Response Service which consists of Norfolk First Support and Swift Response.

We have a vacancy at Grays Fair Court, a Housing with Care site in Norwich. As a Reablement Support Lead, you will work closely with Operational Managers and Reablement Support Workers to support tenants in maintaining their independence and ensure the smooth running of the service.

Key responsibilities
  • Leading shifts, providing practical support to the team
  • Responding to queries that do not require managerial input
  • Coordinating short-notice staff cover, handovers and inductions
  • Overseeing repeat prescription orders
  • Responding promptly to call bells and emergencies, delivering person-centred care, and liaising with medical professionals as needed
  • Acting as the senior on duty in the absence of the Operational Manager, including managing incidents, contacting emergency services, and escalating urgent maintenance issues
  • Serving as a key worker for tenants, gathering feedback, supporting community engagement, contributing to MDT discussions, and reviewing care plans

You will need to have experience of working with people and be able to communicate clearly and appropriately with members of the public, whilst exercising judgement and using your initiative to plan and prioritise your work. You will also need knowledge of the current policy, practice and legislation within Health and Social Care.

Full training will be provided to ensure you are confident and competent in delivering this vital role within our Housing with Care service.

Grays Fair Court is a care establishment made up of 34 Housing with Care flats. All flats are self-contained with bedroom, lounge, kitchenette and bathroom. The Housing with Care tenants also have access to 24 hour assessed and unplanned care.
