Job summary

Civic Recruitment Limited in Islington is seeking a Reablement Coordinator to lead a small multidisciplinary team delivering strengths-based, person-centred reablement at adults' homes. You will manage a caseload, develop and review plans, and coordinate safeguarding and assessment activities to support independence.

In this role you will work with health and social care colleagues, carers and families across Islington to ensure timely transitions and effective service delivery, including

Qualifications

  • GCSE English and Maths or equivalent.
  • QCF Level 5 qualification in Health and Social Care, or willingness to undertake the qualification.
  • Knowledge and understanding of strengths-based approaches and reablement principles.
  • Understanding of safeguarding responsibilities, including Making Safeguarding Personal.
  • Working knowledge of health and social care legislation, including the Care Act 2014, Mental Capacity Act 2005 and other relevant statutory frameworks.
  • Awareness of regulatory requirements for care services, including CQC standards.
  • Willingness to complete Trusted Assessor training to support assessment and provision of equipment and minor adaptations.

Responsibilities

  • Coordinate a caseload of individuals receiving reablement support, ensuring services are personalised, strengths-based and focused on achieving identified outcomes.
  • Complete strengths-based reviews with individuals, carers and professionals to assess progress and maximise independence.
  • Develop, review and update reablement plans, support plans and risk assessments in line with individual needs and goals.
  • Ensure the Care Act 2014, Mental Capacity Act and relevant health and social care legislation are embedded within practice.
  • Identify, manage and escalate safeguarding concerns in line with safeguarding policies and procedures.
  • Work collaboratively with Social Work teams, health professionals, housing services, GPs and other stakeholders to ensure effective and coordinated support.
  • Support individuals and carers to manage risks while promoting independence and positive risk-taking.
  • Monitor changes in an individual's wellbeing, health, circumstances or support needs and ensure appropriate action is taken.
  • Coordinate safe transitions from the reablement service, arranging appropriate longer-term support only where required.
  • Provide effective supervision, guidance and support to Reablement Support Workers, ensuring good practice and a consistent reablement approach.
  • Support quality assurance activities, audits, service improvement initiatives and monitoring of service outcomes.
  • Maintain accurate, timely and professional records, including assessments, reviews, support plans and risk documentation.
  • Use digital systems, applications and IT equipment effectively to manage workload, allocate tasks and maintain service records.
  • Attend and contribute to multi-disciplinary meetings, reviews, supervision sessions and team meetings.
  • Promote equality, diversity, inclusion and dignity in all aspects of service delivery.
  • Maintain high standards of infection prevention and control and comply with health and safety requirements.
  • Work flexibly across a seven-day service, including community settings and individuals' homes, in line with service requirements.

Job Purpose

The Reablement Coordinator will be responsible for coordinating and delivering strengths-based, person-centred support to adults receiving a period of reablement through the Islington Reablement Service.

The role aims to support individuals to regain or maximise their independence, enabling them to achieve their personalised reablement goals and live as independently as possible within their own homes or chosen living environment. The post holder will work with adults with a range of needs, including physical disabilities, sensory needs, learning disabilities, mental health needs and age-related conditions.

Working as part of a multi-disciplinary team, the Reablement Coordinator will manage a caseload of individuals, completing reviews, updating reablement plans and risk assessments, monitoring progress and ensuring support is tailored to individual strengths, outcomes and aspirations. The post holder will coordinate transitions from the service, ensuring individuals are supported to move on successfully within six weeks or sooner where appropriate, with long-term care arrangements only considered where necessary.

The post holder will provide guidance and supervision to Reablement Support Workers, promote a strong reablement ethos, ensure safe and effective service delivery, and work collaboratively with health, social care professionals, carers, families and partner agencies.

  • Experience applying strengths-based approaches in practice.
  • Experience completing assessments, reviews, risk assessments and support plans.
  • Experience supporting informal carers and recognising their needs.
  • Experience supervising or supporting staff teams within a care or support environment.
Skills and Competencies
  • Ability to complete person-centred reviews and support individuals to achieve personalised reablement goals.
  • Strong communication skills, both written and verbal, with the ability to maintain accurate professional records.

Additional Requirements
  • Enhanced DBS clearance required.
  • Ability and willingness to travel across the service area.
  • Ability to work in individuals' homes, including environments where there may be challenges such as hoarding, self-neglect or animals present.
  • Flexibility to work across a seven-day service operating between 8am and 10pm.
