The role of the Reablement Assistant adopts a strength-based approach, supporting and encouraging a person’s independence. Responsibilities include developing therapy plans, providing direct care and support, and signposting to other services. The assistant will deliver care throughout the borough, working with individuals to regain daily life skills and enable independent living in their own homes.

Key duties include assisting individuals to retain independence through person-centred planning, liaising with other professionals to minimize risks, and carrying out medication administration as required.

Salary: £26,409 - £30,060 per annum

Closing date: 29/05/2025

Hours: Part-time, working flexibly over 7 days between 8am and 10pm on a rota. Salary and allowances are paid pro-rata.
