Job description

We are looking for an enthusiastic professional to join the Community Independence Service (CIS) in Hammersmith & Fulham as a Rapid Practitioner in physical health service. Join our dedicated team to help make Hammersmith & Fulham a great place to grow old and deliver high-quality patient care.

As a key member of the CIS, you will develop effective care pathways from hospital to community, building strong relationships with hospital and community partners. We support your clinical and leadership development, aligning with your personal development and appraisal plans.

Our service has achieved excellent results in admission avoidance and reducing hospital stays, and is part of a wider three-borough CIS alongside Westminster and Kensington & Chelsea. We offer mobile working options and support flexible working practices.

Key Responsibilities
  1. Work as an experienced Registered Nurse in Adult Nursing within the Rapid Response Team and Home First Team (H1st).
  2. Provide community-based responsive care to patients with complex needs, ensuring holistic assessments and care pathway planning to prevent unnecessary hospital admissions.
  3. Act as a clinical resource and first responder within a 2-hour response time, facilitating access to community health and social care services.
  4. Make clinical decisions supporting patient choices, working autonomously with support from senior colleagues in primary and secondary care.
  5. Work within service hours from Monday to Sunday, on shifts from 8 am – 8 pm or 10 am – 10 pm across tri-borough sites.
  6. Car driving is essential for this role.
