Job summary

Connected Health is seeking a Rapid Responder to support safe, high-quality home care across the Trust zone. You will work with the Registered Manager to mobilise new services, assist with hospital discharges, and manage care packages while ensuring health and safety compliance.

The role offers a £28,228 salary with mileage paid, sign-on bonus after 3 months, and additional benefits. Flexibility in shifts and ongoing training support career development within a growing homecare service.

Responsibilities

• Supporting the development and mobilisation of new home care services
• Providing cover and support across the allocated Trust zone as required
• Supporting the filling of new care packages and hospital discharges
• Undertaking emergency care cover where required
• Supporting staff shadowing and ensuring Health and Safety policies and procedures are followed
• Delivering care plans and log sheets to clients' homes when required
• Delivering PPE and other essential documentation or supplies
• Travelling between clients' homes, offices and service areas within the allocated Trust zone
