Job Title: Social Worker – Older Adults Mental Health Team

Location: Waltham Forest

Rate: £32 per hour

An exciting opportunity has arisen at Waltham Forest for a qualified Social Worker to join their Older Adults Mental Health Team.

Responsibilities:
  • Work with clients with functional and organic (dementia) mental health diagnoses
  • Practise as a care coordinator within a multidisciplinary team
  • Manage a caseload of 18-20 clients, monitoring and supporting their mental wellbeing
  • Complete and implement Care Act 2014 assessments and support plans
  • Conduct Mental Capacity Assessments and safeguarding enquiries
  • Familiarity with Rio and Mosaic systems is essential
Qualifications and Experience:
  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
