Qualified Social Worker

Transitions

Location: West Yorkshire

We are looking for experienced and passionate Qualified Social Workers to join a Transitions team. As part of this dedicated team, you will work in a busy and supportive environment assessing and working with young adults and their families, intervening where support is needed to provide help at an earlier stage.

The role involves assessment work directly with young adults and their families, partnering with agencies including the police and health services to ensure the best outcomes with sustainable positive change. The service maintains strong links with our Integrated Prevention and Earliest Help colleagues to promote local community support when social care intervention is no longer required.

Working in accordance with social care legislation and local government requirements, and guided by the values and principles of social care, you will support the delivery of person-centred, safe, and effective social care services. Using your professional knowledge and experience, you will make assessments and decisions about appropriate interventions. Applying your significant social work experience and excellent communication skills, you will establish and maintain relationships with service users and other agencies to meet identified needs.

You will be committed to ensuring positive and sustainable results from your interventions. Using best practices, you will strive for the best outcomes for children. Most importantly, you will have a passion for supporting children and their families to thrive and flourish. Our priority is to protect the most vulnerable and to work efficiently.

Perfect candidate will be:

  • Qualified Social Worker registered with Social Work England.
  • SW, CSS, DipSW, or Social Work Degree; creative, flexible, and forward-thinking.
  • Experience working in a Social Services Directorate or other statutory or voluntary agency.
  • Constantly striving for continuous improvement to provide the best outcomes for individuals and families.
  • Excellent mediation and negotiation skills on behalf of service users.
  • Experience working with a range of client groups within an agency.
  • Highly skilled at building meaningful relationships with children and families to engage them in assessments and plans.
  • Committed to providing high-quality services.
  • Experience working in a multi-disciplinary, interagency environment.
  • Knowledge of legislation relating to general social work practice.
  • 4+ years qualified.

In this role you will:

  • Conduct comprehensive assessments of risks, protection, and social care needs.
  • Manage an agreed caseload.
  • Use specialist communication skills to achieve optimal outcomes for service users.
  • Develop individual care plans that respect diversity and promote dignity and empowerment.
  • Offer advice and support to unqualified staff and students for development.
