Qualified Social Worker, Team Manager, Hospital Discharge Team

Pay rate: £46 per hour

Contract role

Vitalis is recruiting an experienced Social Worker to work as a Team Manager in a Hospital Discharge Team in Stockport.

Stockport (SMBC) requires a strong and dynamic team manager to work within ARCH, our hospital discharge team at Stepping Hill Hospital.

The manager must have a minimum of 3 years’ experience. A minimum of 4 days on-site is required.

The manager should have thorough knowledge of legislation (Care Act, safeguarding, MCA, BIM, etc.), managing and supervising social workers and unqualified staff to support hospital discharges, and experience with pathway work. The role is based at Stepping Hill Hospital and Regent House, Stockport.

Experience in managing a large team, including social workers, ASYE, and apprenticeships, is essential.

Working hours are 8:30 am - 5:00 pm within a busy environment, supporting staff to ensure safe and timely discharges.

Experience working with partners such as ICB, health services, and third-party sectors is required.

The ideal candidate is reliable, flexible, dynamic, and able to perform under pressure.

Responsibilities include supervising social workers, chairing safeguarding meetings, COP work, MDTs, and other professional meetings. The role requires on-site presence at the hospital and other bed base sites.
