What will your responsibilities be?
As a Qualified Social Worker in the Continuing Healthcare Team at Southwark, you will conduct CHC and Care Act Assessments and Reviews, ensuring a person-centred, strengths-based approach. You will collaborate within a multi-disciplinary team, manage a caseload of individuals with complex needs, and develop care plans that promote independence and well-being. Your role includes providing crisis intervention, safeguarding, and empowering clients to advocate for themselves. You will engage in professional development and ensure compliance with statutory requirements and best practices, aiming to deliver high-quality, sustainable social work services.

Qualifications and Experiences:
You will conduct CHC Assessments and Reviews, ensuring a thorough, person-centred, and strengths-based approach. You will collaborate effectively within a multi-disciplinary team to deliver optimal care outcomes. Additionally, you will perform Care Act Assessments and Reviews, upholding the principles of the Act in all professional activities.
You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
