Job Description:

An exciting job role has recently become available in Rochdale. They are looking for a new member for their Adults Team.

What will your responsibilities be?

In this role, you will provide person-centred assessments of need at a level of complexity appropriate to the role, such as addressing multiple social or health needs and managing complex care and support arrangements as allocated by your line manager. You will work with people in crisis, where urgent support arrangements are needed, and make decisions regarding mental capacity and/or the Mental Health Act. Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements. Using active listening skills, observation, and communication, you will develop supportive and therapeutic relationships with adults, their families, and informal networks, exploring their situations and collaboratively finding ways to move forward.

Qualifications and Experiences:

To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
