Pay rate £38 per hour

Vitalis is recruiting for a Qualified Social Worker to work in an Older People’s Team in Lincolnshire. We need 2 social workers to cover the South area of Lincolnshire. The office base is flexible, such as Sleaford, Spalding, Grantham, Stamford, and team members should be able to work across area teams when needed.

Requirements include experience with Mosaic, adult needs assessments, care planning, commissioning, working with adults with physical disabilities and frailty, mental capacity assessments, strength-based approaches, signposting, direct payments, CHC, DSTs, and adults safeguarding.
