The Person
  • Must be a qualified Social Worker with experience working with adults
  • Current DBS ideally on the update service
  • Registered with Social Care Wales
  • A basic understanding of the Welsh language is desirable
The Role

Accessing Personal Social Services

  • Engage in structured conversations with those making initial enquiries and evaluate the information provided
  • Provide information, advice, and signposting to other services
  • Apply service access criteria to determine referral appropriateness
  • Assess urgency and priority of referrals
  • Make initial enquiries with other agencies or contacts
  • Record referral information accurately and fully according to departmental procedures
  • Ensure timely transfer of referral information
  • Safeguard the welfare of vulnerable persons at immediate risk of harm
  • Evaluate the nature of potential needs based on referral information and previous records
  • Identify risks of abuse, failure to protect, harm to self or others, and assess intervention needs; undertake training to act as a Best Interest Assessor under the Mental Capacity Act 2005
  • Arrange appointments and visits for assessments
  • Engage in direct work with service users and carers for initial assessments
