Job Title: Social Worker – Hospital Discharge Team

An exciting job role has recently become available in Liverpool. They are looking for a new member for their Hospital Discharge Team.
Responsibilities include: managing a complex caseload with supervision, preparing reports, representing clients at Court hearings and Court of Protection applications, making complex decisions, managing risk with professional judgment and evidence-based tools, effective communication in high-risk situations, representing the department in multidisciplinary forums, conducting direct work with service users, carers, and families following safeguarding policies, assessing and navigating care for individuals with social care needs, collaborating with health agencies, planning and delivering services within financial constraints, monitoring support plans, and participating in safeguarding and capacity assessments.
To be successful, you must have Social Work England registration, be eligible to work in the UK, hold a degree in Social Work, and have post-qualified experience.
