Qualified Social Worker, Learning Disability Team

Pay rate: £33.94 per hour

Contract role

Vitalis is recruiting an experienced Social Worker to work in a Learning Disability Team in Hackney.

Role specific responsibilities:

1. Record actions and decisions clearly and robustly on the Mosaic system.
2. Work as part of a multi-agency team providing seamless health and social care services.
3. Provide direct advice and assistance, offering preventative support for adults with Learning Disabilities.
4. Respond to urgent matters raised on duty, using a range of resources to promote the safety and wellbeing of service users with learning disabilities.
5. Carry out assessments, reviews, and visits as required, especially in emergencies.
6. Be the first point of contact for safeguarding concerns, following up to ensure the safety of service users, liaising with internal and external colleagues as appropriate.
7. Ensure adherence to risk management in social work practice.
8. Apply the principles of the MCA Act where appropriate.
9. Deliver high professional standards of social care practice, including quality interventions, decision-making, performance management, and adherence to the Standards of Social Work England.
