Job Title: Qualified Social Worker – Adults/Combined Locality Team
Location: Knowsley
Rate: £31.15 per hour
An exciting job role has recently become available in Knowsley. We are looking for a new member for their Adults/Combined Locality Team.
What will your responsibilities be?
You will provide safe care and support of adults ‘at risk’ ensuring all their basic needs identified in their support plan and risk assessments are fully met. You will alsoprovide person-centred quality care that is tailored to the needs and aspirations of each individual. This personalised approach must ensure that individual outcomes are met and everyone is treated with dignity, respect and compassion.

Support the safe delivery of care plans, ensuring individual needs are met and CQC standards maintained. Assist clients with personal care tasks such as dressing, bathing, eating, and medication as outlined in care plans. Monitor service user progress, identify triggers for challenging behaviour, and update managers on changes. Provide emotional support to clients, raising significant concerns to the manager. Contribute to planning daily activities that promote service users’ well-being. Administer and record medication, including specific personal care tasks, following council policies. Maintain accurate and updated records to ensure compliance with information security protocols. Communicate professionally with all stakeholders involved in care delivery, attending meetings as required. We are looking for a Social Worker with Social Work England registration, eligibility to work in the UK, a full UK license, post-qualified experience, and a degree level or equivalent in Social Work.
