Qualified Social Worker
Hospital Discharge
Location: Merseyside
Pay rate: £39 per hour

Our client is looking for qualified adult social workers to join the Hospital Discharge team. Candidates should have previous experience in adult social care, a good understanding of relevant legislation such as the Care Act 2014, Mental Capacity Act 2005, and others. The role involves screening safeguarding referrals, case holding complex safeguarding cases, and working within a specific area to carry out assessments, safeguarding, multi-agency work, and duty functions. This is a fully office-based role.

You will be responsible for managing a team of social workers and professionals, ensuring statutory and legislative duties are met, safeguarding vulnerable individuals, and promoting positive outcomes. The role requires collaboration across teams, adherence to social care legislation, and professional judgment in assessments and interventions. The ideal candidate will have at least 4 years of qualified social work experience, registration with Social Work England, and excellent communication skills.
