Job Title: Qualified Social Worker – Adults MASH Team
Location: Hillingdon
Rate: £42.28

An exciting job role has recently become available in Hillingdon. We are looking for a new member for the Adults MASH Team.

Responsibilities:

You will work within the context of the Care Act 2014 to provide person-centred Social Work intervention, strength-based work, and a preventive approach, enabling service users and carers to optimise and maintain their independence and wellbeing. You will work with clients to produce a reablement plan which will help service users identify and achieve their goals relating to social care tasks such as washing, dressing, showering, meal preparation, and engaging in community activities. You should be able to carry out joint working with therapists and have knowledge of assistive technology.

Qualifications and Experience:
