About the Role:

Bristol City Council is seeking an experienced and dedicated Social Worker to join our Discharge to Assess (D2A) team. This is a vital role supporting adults as they transition from hospital to home or community settings, ensuring timely and safe discharges while promoting independence and person-centered care.

Key Responsibilities:

  • Undertake strengths-based assessments under the Care Act 2014.
  • Collaborate with health professionals, families, and providers to develop effective discharge plans.
  • Support service users in accessing appropriate community services, care packages, and reablement.
  • Maintain accurate and timely case notes and documentation.
  • Safeguard vulnerable adults and uphold best practices in line with council and legislative standards.
