Job Description:

Carry out social work tasks to a professional standard in accordance with policies of the Social Services Committee and instructions of the Service Unit Head. Ensure support plan reviews are timely and person-centered, evidencing need for ongoing support.

Undertake comprehensive assessments, contribute to risk assessments and management plans for adults, and undertake care coordination for designated caseloads following Dorset Healthcare policies.

Conduct carers assessments, provide information and advice to adults and their carers, promote self-care, and support the use of personal budgets and direct payments.

Perform best interest assessments considering the Mental Capacity Act, and assist adults in understanding the support the Council can offer, ensuring resource use aligns with person-centered outcomes.

Act as an Approved Mental Health Professional (or undertake training), make hospital admissions under the Mental Health Act, and act as a Safeguarding Adult Practitioner (or undertake training).

Train as a Deprivation of Liberties Assessor, act as a Care Manager as required, and maintain essential records and participate in team and official meetings.

Manage caseloads, provide consultancy services, engage in professional development, and prepare reports for legal proceedings.

Perform liaison roles such as dual diagnosis link worker, and undertake other duties as required, complying with all relevant policies and legislation, including SWE registration where applicable.
