Responsibilities include:

• Engaging in a fast-paced, multi-disciplinary environment, working closely with health colleagues and community agencies.
• Demonstrating knowledge of hospital pathways to support patient care.
• Conducting Care Act Assessments, Mental Capacity Assessments, and chairing Best Interest Meetings.
• Carrying out timely reviews to ensure ongoing support for patients.
• Applying a Multi-Disciplinary Team (MDT) approach for safe and efficient hospital discharges aligned with service users' needs.

Qualifications and Experience:
