Job Description:

Requirements:

• To work within the context of the Care Act 2014, to provide person-centred Social Work intervention, strength-based work, preventive approach, and enabling service users and carers to optimise and maintain their independence and wellbeing.
• Work with clients to produce a reablement plan that helps service users identify and achieve their goals related to social care tasks such as washing, dressing, showering, meal preparation, and engaging in community activities.
• Ability to carry out joint working with therapists and knowledge of assistive technology.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
