We are currently seeking an experienced and compassionate Adults Social Worker to join our dynamic team.

About the role:

• To be able to undertake Care Act Assessments, Mental Capacity assessments & Best Interest outcomes. Complete Community DOL assessments, experienced with CHC checklists and DST's.
• Experienced social worker with a good track record to operate from a Strengths Based perspective, demonstrating Least Restrictive Practice. Support the team duty function by triaging referrals into the team and effectively prioritising day-to-day case management activities.
• Confident in working with professional partners in an MDT approach.
