• Position: Assessment Social Worker
• Salary: £37ph

Bolton is looking for a Social Worker for the Neighbourhood team, working with adults aged 18+ with various needs (excluding Learning Disabilities, which are addressed by a different team).

The candidate will collaborate with health professionals such as district nurses and therapists.

Candidates should have experience and knowledge of:

• Care Act assessment
• Strength-based support planning
• BIA and capacity reviews
• Safeguarding
• At least 2 years of experience within a specific neighbourhood team, or 3+ years of relevant experience
• Must be local to Bolton and able to commit to working in Bolton, as the role is hybrid but requires the ability to come to the office or complete a visit at short notice
