Qualified Social Worker – Adults Mental Health

Location: Cheshire
£30 per hour (Agency)
Contract Type: Locum | Hybrid Working Available

Are you a Qualified Social Worker with a strong background in adult mental health and hospital discharge? We’re looking for experienced and motivated practitioners to join a Mental Health & Hospital Discharge Team in Cheshire, supporting adults to live safely, independently, and with dignity. You’ll be working across community and hospital settings, ensuring that people receive timely, strengths-based support tailored to their needs.

What You’ll Be Doing:

• Completing Care Act assessments to support safe and timely hospital discharges
• Carrying out mental capacity assessments and contributing to best interest decisions
• Managing a caseload including safeguarding enquiries, complex care planning, and reviews
• Liaising with hospitals, community services, families, and other professionals
• Ensuring service users receive holistic, appropriate, and empowering support

Who We’re Looking For:

• Qualified Social Worker registered with Social Work England
• Proven experience in Adult Social Care, ideally in mental health or hospital discharge teams
• Strong working knowledge of:
  • Care Act 2014
  • Mental Capacity Act 2005
  • Mental Health Act 1983 (as amended)
  • Human Rights Act and safeguarding legislation
• Skilled in multidisciplinary working and confident managing complex cases
• Committed to strengths-based practice, inclusion, and independence

• A recognised social work qualification (DipSW, CQSW, BA, MA in Social Work)
• Excellent communication and decision-making skills
