About the role:

Wirral Council is seeking an experienced Social Worker to join their Adult Community Team on a locum basis. This is a fantastic opportunity for a passionate and skilled practitioner to support adults within the community, promoting independence, dignity, and person-centered care.

Key Responsibilities

Work with adults with a range of needs including physical disabilities, older adults, and those with complex needs.Carry out Care Act assessments, Mental Capacity Act assessments, and safeguarding enquiries.Develop care and support plans in line with statutory requirements.Work collaboratively with health professionals, families, and carers
