What you will do:

1. Work within the Multi-Agency Safeguarding Hub to coordinate care for vulnerable adults.
2. Assess and implement a range of social care services, ensuring compliance with relevant legal frameworks and standards.
3. Provide supervision and support to newly qualified social workers and Care Navigators within the service.
4. Carry out complex needs assessments and create comprehensive support plans.
5. Collaborate with multi-disciplinary teams and external services for effective signposting and referrals.
6. Engage in safeguarding processes and ensure adherence to best practice and legal requirements.
7. Conduct Best Interest assessments in relation to Deprivation of Liberties.
8. Effectively communicate sensitive information to stakeholders and collaborate effectively with community forums.
