Adult's Social Worker - Mental Health Team
Location: Rochdale Council
Rate: Up to 30.00 per hour
Contract: Full-time, Locum

About the Role:
As a Social Worker in the Mental Health Team, you will support adults with complex mental health needs, ensuring their rights, dignity, and wellbeing are prioritised. You'll carry out assessments under the Care Act 2014, coordinate care plans, and work in partnership with health professionals, service users, and their families to promote recovery and independence.

You may also be involved in safeguarding investigations, risk management, and applications under the Mental Health Act and Mental Capacity Act.

Key Responsibilities:

  • Undertake strength-based assessments and reviews
  • Create and implement personalised care and support plans
  • Work collaboratively with mental health professionals, housing, and voluntary services
  • Manage risk and promote safe discharge planning from hospital or inpatient settings
  • Conduct Mental Capacity Act assessments and contribute to Best Interest decisions
  • Safeguard adults at risk of abuse or neglect
  • Maintain accurate case records in line with statutory requirements

Requirements:

  • Social Work England registration
  • A recognised Social Work qualification
  • Enhanced DBS
  • Previous experience in adult social care, ideally in mental health
