Job Title: Qualified Adults Social Worker – Hospital Discharge Team
Location: Surrey
Rate: Up to £38 per hour

Are you passionate about making a difference in the lives of individuals transitioning from hospital to home? The East Surrey Hospital Discharge team is seeking a dedicated and compassionate Hospital Discharge Social Worker to support the community. As part of our Hospital Discharge Team and Community Home First Hub Service, you’ll play a crucial role in ensuring safe and effective discharges.

What will your responsibilities be?

You will undertake Care Act 2014 assessments, focusing on the person’s well-being and adopting a person-centered, strengths-based approach. You will empower individuals by promoting choice, control, and independence through reablement approaches. Additionally, you will collaborate with other professionals, source provisions, and safeguard vulnerable adults.

Qualifications and Experiences:

You must be registered with Social Work England, be eligible to work in the UK, and hold a full UK driving licence. Additionally, you need to have post-qualified experience and possess a degree level or equivalent qualification in social work.
