In this rewarding role, you'll work across community-based adult services, carrying out complex casework and safeguarding interventions that protect the most vulnerable while promoting dignity, empowerment, and independence.
What You'll Be Doing:As part of the Integrated Neighbourhood Team, you will:
- Carry out Care Act Assessments and Mental Capacity Assessments
- Lead on safeguarding enquiries and manage complex adult protection cases
- Participate in duty rotations, responding to urgent referrals and risk
- Work collaboratively with health, housing, and voluntary sector partners
- Champion person-centred care planning and positive risk-taking
- Uphold key legislation including the Care Act 2014, MCA 2005, MHA, and Human Rights Act
About You – The Ideal Candidate:We’re looking for a confident and compassionate practitioner who thrives in a fast-paced, multi-agency environment and puts the individual at the heart of every decision.You’ll bring:- ️ Registration with Social Work England
- ️ A recognised Social Work qualification (DipSW, CQSW, Degree or equivalent)
- ️ 4+ years’ experience in adult social care
- ️ Strong working knowledge of relevant legislation and frameworks
- ️ Excellent communication, assessment, and case management skills
- ️ A proactive, strengths-based, and culturally sensitive approach
- ️ Experience in safeguarding, MCA, DoLS, and multi-disciplinary working
Key Responsibilities Include:- Making professional decisions using sound risk assessments and safeguarding principles
- Managing your own caseload efficiently and independently
- Developing tailored care and support plans that reflect individual needs
- Offering support to unqualified staff and students
- Using data, systems, and insight to improve outcomes
