Are you a Qualified Adult Social Worker ready to make a real impact in a Hospital Discharge setting? We’re working with a forward-thinking local authority in Merseyside looking for experienced professionals to join their dedicated Adult Services team.

About the Role:
You’ll play a key role in supporting safe and effective hospital discharges, ensuring the needs of vulnerable adults are met while reducing risk and promoting independence.
Working in line with social care legislation, you’ll carry out strength-based assessments, create personalised care plans, and lead interventions to secure the best outcomes. You’ll collaborate closely with hospitals, community services, and families—putting people at the heart of your practice.

What We’re Looking For:
We’re seeking confident, compassionate professionals who bring:

• Social Work England registration
• 4+ years’ experience in Adult Social Care (ideally 2–3 in hospital discharge or similar)
• A SW, CSS, DipSW or Social Work Degree
• A commitment to best practice, dignity, and empowerment
• Strong skills in mediation, assessment, and multi-agency collaboration
• Knowledge of current legislation and safeguarding frameworks

In This Role, You Will:

• Manage a caseload of complex discharges
• Carry out risk assessments and devise care plans
• Use specialist communication skills to achieve the best outcomes
• Provide guidance and mentoring to unqualified staff and students
