Job Title:Social Worker – Older Persons Team
Location:Wolverhampton
Rate:Up to £33.20 per hour
Working Hours:Full-time (37 hours per week) or part-time options available
Contract:Initial 3-month contract with scope to extend
An exciting job role has recently become available in Wolverhampton. They are looking for a new member for their Older Persons Team. As an Adult Social Worker, you will play a crucial role in supporting vulnerable adults within our community. You’ll work closely with individuals, families, and other professionals to promote independence, well-being, and social inclusion. Your compassionate approach and strong assessment skills will contribute to positive outcomes for those you serve.
What will your responsibilities be?
Conduct comprehensive assessments of individuals’ needs, considering physical, emotional, and social factors. Develop personalized support plans that empower clients to achieve their goals. Collaborate with health professionals, care providers, and community resources to coordinate services. Identify and respond to safeguarding concerns promptly. Assess risks related to vulnerable adults and implement appropriate interventions. Work within legal frameworks to ensure the safety and well-being of service users. Provide emotional support, counseling, and advocacy. Assist clients in accessing relevant services, benefits, and housing. Facilitate group sessions and workshops to enhance social skills and resilience. Liaise with healthcare professionals, housing officers, police, and other agencies. Attend case conferences and multidisciplinary meetings. Advocate for clients’ rights and needs within the wider system.
