An exciting opening has arisen at the Princess of Wales Hospital, Bridgend, for a social worker to join the dynamic Hospital Social Work Team within the Early Intervention & Prevention Hub.

Responsibilities:

• Conduct comprehensive assessments to determine individuals’ needs in accordance with the principles of the Social Services and Wellbeing Act.
• Develop and implement care plans in collaboration with multidisciplinary teams.
• Facilitate access to community resources.
• Conduct capacity assessments and address best interests.
• Advocate for individuals’ rights and services, ensuring ethical and legal compliance.

What we are looking for in a Social Worker:

• Social Care Wales registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
