Bromley are looking for a Qualified Social Worker to join a locality team on a long-term contract until 2025!
The role will involve providing an assessment service to individuals, their carers and other agencies in accordance with Care Act (2014) criteria, reviewing the needs of individual service users and carers.
The ideal candidate will have some experience of the above, developing support plans, undertaking assessments and participating in Multi-Disciplinary meetings.

Responsibilities:
– Ensure the wellbeing ethos is upheld when working with vulnerable adults and their carers.
– Participate in duty operations, responding to needs including Safeguarding enquiries and Best Interest Assessments (BIA) as necessary.
– Conduct assessments for individuals, carers, and agencies according to Care Act (2014) criteria.
– Coordinate care and support plans for individual service users and carers in collaboration with colleagues and representatives from voluntary and private agencies.
– Provide supervision and support to staff as per agreements with Line Manager and Council procedures.
– Offer appropriate support and advice to service users and carers.
