Job description

An exciting opportunity to join The Hampshire and Isle of Wight Healthcare Children's Public Health Nursing Service. This is a Specialist Public Health Nurse role for children, young people, and families who may be most vulnerable to health inequalities, ensuring access to the Healthy Child Programme. You will be part of a dynamic team delivering high-quality services, supporting Hampshire's CYP to reach their full potential.

The post holder will support an accessible universal school nursing service to Children and Young People (CYP) who are most vulnerable outside mainstream education and/or belong to vulnerable groups. This may include young carers, those not in education or training, electively home-educated, asylum seekers, homeless, and gypsy Roma travelling communities. The role involves developing and providing targeted, personalized care to CYP aged 5-19 years to improve health and educational outcomes.

The Public Health Inclusion Nurse will collaborate with the Public Health 5-19 Area Manager, clinical team leads, and stakeholders to enhance standards, promote good practice, and ensure clinical effectiveness in delivering the Healthy Child Programme. The role includes providing professional advice, safeguarding, and clinical input within their area of practice.

This position involves working with vulnerable families, children, and young people, leading the delivery of the Healthy Child Programme (5-19) to improve child health outcomes and early identification of families in need. The role includes outreach services, offering time-limited self-management support, and working across health and social care boundaries, implementing evidence-based standards and policies for those less able to attend mainstream services.
