Psychosocial Team Leader - HMP Feltham B

Clinical Responsibilities: To work as a member of the Wellbeing team, which provides a range of specialist services to substance users and those with psychological and mental health needs, including harm minimisation approaches, motivational interviewing, relapse prevention, care planning, health education and detoxification support, mindfulness and groupwork. To make continuous assessments of the psychological, emotional, social, and rehabilitative needs of young people and young adults, considering the needs of family, local community and sentence plan, with regular evaluation of care plans. To provide case management to a defined caseload of clients and collaborative working with the Substance Misuse Clinical Team Leader and the Prescribers To use agreed and established outcome monitoring tools and any other relevant quality measures. To manage the transfer of individuals between prisons and into the community. To counsel individuals about their substance use and/or harmful behaviours, using motivational interviewing and other evidence-based interventions. To ensure full risk assessment and risk management is delivered effectively. To understand and implement clinical governance. To ensure the highest quality of service is delivered by following local and national quality and performance standards, guidance, procedures, and practice guidelines. To ensure that all work is performed in accordance with Trust policies and procedures, including Incident Reporting. To support the wider service with additional interventions as appropriate and with relevant training, e.g. initial clinical assessment, second signatory for controlled drugs, urine testing, dry blood-spot testing, NDTMS reporting, Drug strategy reporting and data for equality/diversity of service users on site requesting support from the service, Perform CHAT assessments etc. To support the integration of the psychosocial team into the wider Health and Wellbeing team, ensuring attendance at HWBT meetings, multidisciplinary formulation meetings, and HC governance and prison meetings as delegated by HWBT manager. Work closely with the consultant psychologist to promote the delivery of secure stairs in Feltham A and to deliver the principles of Secure Stairs as required. Coordinating and attending all reviews: 5 days review, 28 days review and 13 weeks review. Ensuring monthly data collection for HJIPs, OST and Naloxone Monitor Notification Alert of new arrivals on OST and completing the PNAP process on release to community (PNAP- Probation Notification Actioning Project)
