Job description

Are you a Mental Health Nurse (RMN) seeking a fresh challenge in Psychiatric Liaison Services? We have an exciting opportunity for a clinician to join our dynamic and supportive team at Yeovil District Hospital.

This role offers a unique chance to work alongside experienced Psychiatric Liaison professionals, gaining exposure to a wide range of clinical presentations and developing specialist skills in acute mental health care. You'll be supported through the Trust's preceptorship programme (if applicable), and our internal PLT development pathway, which includes allocated training days and a clear route to Band 6 progression.

You'll be part of a multidisciplinary team bridging the gap between physical and mental health, assessing patients who present to hospital with complex needs and helping to shape their onward care plans.

Main duties of the job

As a RMN within the Psychiatric Liaison Team, you will:

  • Deliver high-quality, evidence-based mental health care in a fast-paced acute hospital setting.
  • Assess and manage risk, develop care plans, and provide interventions tailored to individual patient needs.
  • Liaise with internal and external services including Home Treatment Teams, Adult Mental Health Teams, and community services to ensure continuity of care.
  • Be involved in the Trust's clinical governance activities and uphold standards of confidentiality, safeguarding, and equality.
  • Receive regular supervision and appraisal, with opportunities to act as a role model and support students or junior staff.
  • Be expected to travel within the Trust area and occasionally work in other locations to meet service demands.

Job responsibilities
  • Conduct comprehensive mental health assessments for patients presenting to the Acute Trust with suspected or actual mental health issues.
  • Provide specialist advice and support to acute hospital staff to manage mental health presentations effectively.
  • Work collaboratively with a range of professionals across health and social care to ensure safe and effective discharge planning and admission avoidance.
  • Participate in the PLT development programme, including training and supervision to support progression to Band 6.
  • Work rotational shifts, including evenings and weekends, and be flexible to support service needs across the Trust.
  • Maintain accurate clinical records and contribute to audit and service evaluation activities.
