A regional health service provider in Wales seeks a project manager to lead initiatives in health and social care integration. The ideal candidate will be responsible for project planning and delivery, supporting governance, and engaging stakeholders to improve health outcomes. Strong communication and autonomy in managing workloads are essential for success in this role.
This is an exciting opportunity to join a small, dynamic team of transformation professionals working across the Health Board, Local Authority and Third Sector partners. The role is central to driving the integration of health and social care services in Pembrokeshire. You will work closely with a wide range of stakeholders to support project delivery, governance, and reporting across a portfolio of initiatives aimed at improving health and wellbeing outcomes for our population. You will be expected to lead on the planning, implementation and delivery of specific projects relating to innovation and transformation and also provide project support to the Directorate and Project Manager in the planning and delivery of programmes. You must be able to communicate effectively in order to build good working relationships and be confident in working autonomously, prioritising your own workload in order to meet strict deadlines. You will also be expected to assist in audits to ensure excellent data quality.
