Job summary

Reading Borough Council is seeking an experienced and ambitious Project Manager – Neighbourhood Working to lead a major transformation that will reshape how health, care and community services work together across Reading.

You will implement integrated neighbourhood working with partners across Primary Care, Community Health, Mental Health, Adult Social Care, Public Health and the voluntary sector, delivering outcomes through prevention, integrated care and care closer to home.

Qualifications

  • Experience delivering complex transformation across health and social care sectors.
  • Experience across Integrated Care Systems and across health, social care or local government services.
  • Strong stakeholder management skills with ability to influence at senior levels.
  • Degree-level qualification and recognised PM training required.

Responsibilities

  • Lead design, mobilisation and implementation of complex neighbourhood working projects.

We are looking for an experienced and ambitious Project Manager – Neighbourhood Working to lead a major programme of transformation that will help reshape how health, care and community services work together across Reading.

This is an exciting opportunity to play a leading role in implementing integrated neighbourhood working, bringing together partners across Primary Care, Community Health, Mental Health, Adult Social Care, Public Health, voluntary and community organisations and acute services.

About The Role

Reporting to the Integration Programme Manager, you will lead the design, mobilisation and implementation of complex, system-wide neighbourhood working projects.

You will work with senior leaders and partners across the health and care system to develop sustainable neighbourhood models, coordinate multi-agency activity and ensure projects are delivered to agreed timescales, budgets and quality standards.

You Will
  • Lead the implementation of integrated neighbourhood working across Reading.
  • Develop and coordinate multi-agency teams across health, social care and voluntary sector partners.
  • Manage project plans, risks, dependencies, budgets and milestones.
  • Work with senior leaders, clinicians, commissioners and operational managers to drive transformational change.
  • Use population health management data and community insights to support neighbourhood priorities.
  • Produce reports, presentations and governance updates for senior partnership boards and stakeholders.
  • Support the delivery of wider health and care priorities including prevention, demand management and improved hospital discharge pathways.
About You

We are looking for an accomplished project or programme professional with experience of delivering complex transformation in a multi-agency environment.

You will have experience of working across health, social care and/or Integrated Care Systems, with a strong understanding of partnership working and the challenges of delivering change across organisational boundaries.

You Will Have
  • A recognised project or programme management qualification or training.
  • Experience working within Integrated Care Systems and across health, social care or local government services.
  • A successful track record of delivering large-scale transformation projects.
  • Strong stakeholder management skills with the ability to influence at senior levels.
Experience Managing Project Budgets, Risks, Governance And Benefits Realisation.
  • Excellent analytical, organisational and communication skills.
  • A passion for improving outcomes for residents through collaborative working and innovation.
  • A degree-level qualification or equivalent experience, together with training or qualifications in recognised project or programme management methodologies, is essential.
