Join NHS as a Proactive Care Assistant to support vulnerable patients in managing their complex health conditions. This evolving role involves case finding, administrative tasks, and collaborating with multi-disciplinary teams to enhance patient independence and quality of life.
Proactive Care is an NHSLong Term Plan that aims to provide proactive and personalised support for atargeted subset of individuals living with multiple long-term conditions. Thisservice supports adult patients and carers with complex needs who could benefitfrom proactive Multi-Disciplinary Teams (MDTs) in local communities to be ableto live independently for longer and to reduce avoidable hospital admissionsand GP visits.
TheProactive Care Assistant will have Administration Assistant responsibilities. Thisis an evolving and exciting role and therefore the post holder shoulddemonstrate a flexible and positive attitude, to change with the needs of theservice and service users.
TheProactive Care Assistant will proactively case find and identify patients whoare most vulnerable from admission and/or health deterioration and work closelywith the therapy team to support patients to manage their health conditions andto achieve the best quality of life and independence possible through robusthealth assessments and care planning.
Beresponsible for daily proactive case finding of patients on EMIS and ConnectedCare to identify appropriate patients for a proactive care assessment.
Toparticipate in the smooth running of the proactive care service by undertakingadministrative tasks e.g. making appointments, telephone calls and otherclerical activities.
Beresponsible for arranging, attending and taking minutes for Multi-DisciplinaryTeam meetings.
Be responsible for completion of actions followingMDT which may include generating letters and tasks to patients GP.
To maintainhigh quality, contemporaneous records and assist the therapists and practitionersas necessary with data collection and analysis.
Tobe able to use good verbal and non-verbal communication skills with patientsand relatives, carers and colleagues including those with communication difficulties,adapting communication as required to meet the needs of the patient or carer.
Dealingwith patients, relatives and colleagues queries both in person and on thetelephone confidentially, courteously and efficiently.
Beresponsible for logging and making referrals.
ConfidentIT skills, to include Microsoft Office, Outlook and Excel.
Tosupport seasonal projects as requested by a manager.
To carryout any other reasonable duties as requested by a manager to ensure the qualityof the service.
