Qualifications
  • PT license in the State of Idaho
  • Current CPR certification
  • Valid Driver's License and Auto Insurance
  • Home health and hospice experience preferred
Essential Job Functions/Responsibilities
  1. Provide physical therapy services according to a physician’s plan of care, including assessments, evaluations, and participation in care planning.
  2. Evaluate the home environment and assess muscle strength, mobility, gait, and range of motion to determine rehab potential.
  3. Direct physical therapy treatments and instruct patients and caregivers on therapeutic appliances.
  4. Prioritize patient needs and report treatment responses or condition changes to the physician.
  5. Initiate therapy programs and educate family or caregivers, participating in case conferences.
  6. Train patients in prosthetic device use when applicable.
  7. Identify additional home health service needs and refer patients as necessary.
  8. Prepare and submit clinical and progress summaries, participate in discharge planning.
  9. Provide consultation and in-service education to home health staff and peers.
  10. Supervise Physical Therapy Assistants per organizational and state regulations.
  11. Adhere to documentation, care standards, and professional conduct policies.
  12. Instruct, supervise, and evaluate home health aides when therapy is the sole skilled service, biweekly.
