Job summary

Decypher is seeking an LPN/LVN to obtain vital signs, perform portions of physical exams, administer medications, and document care. This PRN role is onsite and supports the health and readiness of America’s service members, veterans, and families.

Education: Associate's degree in nursing (minimum) or BSN; Licensure: full, current, unrestricted RN license; Certifications: BLS; Experience: 24 months in a primary care/clinical setting; U.S. citizenship required.

Responsibilities

• Collect vital signs and age-appropriate measurements and tests.
• Administer medications via oral, topical, inhalation, and IM routes.
• Assist providers during examinations and procedures.
• Document examinations, treatments, and responses in medical records.
• Coordinate scheduling and use EMR systems such as MHS GENESIS and Essentris.
• Recognize care issues and respond to emergencies with basic life support.

Job Summary: The Licensed Practical/Vocational Nurse (LPN/LVN) will be responsible for obtaining vital signs and performing portions of physical examinations, administering medications via various routes, and documenting medical examinations and treatments. The role also includes preparing patients for diagnostics, assisting providers with procedures, and maintaining patient records. This role is PRN.

Required Qualifications:

• Education: Associate's degree in nursing (Minimum) or Bachelor of Science in Nursing (BSN)
• Certifications:
  • Basic Life Support (BLS)
• License: Must hold a full, current, and unrestricted license in a State, Territory, Commonwealth, or the District of Columbia.
• Experience: Have at least 24 months of experience as a Registered Nurse within the last 36 months in a primary care / clinical setting.

U.S. Citizenship is required

Key Responsibilities:

• Collect vital signs and completed age-appropriate physical measurements and diagnostic tests, including ECGs, pulse oximetry, visual acuity, and fetal doppler tones.
• Administer prescribed medications through oral, topical, inhalation, and intramuscular routes while monitoring for adverse reactions.
• Assist providers during examinations, treatments, and clinical procedures, ensuring proper patient preparation and positioning.
• Document examinations, treatments, procedures, and patient responses accurately in electronic and hard-copy medical records.
• Recognize patient care issues, reported observations promptly to the healthcare team, and respond to emergencies with basic life support measures.
• Managed patient scheduling, intake, registration verification, and appointment coordination using MHS GENESIS, Essentris, ASIMS, and MiCare.

Work Environment & Schedule: PRNOnsite, Monday- Friday from 7:30- 4:30 with a 30-minute lunch break.

Compensation & Benefits: $ 25.00 per hour

Physical requirements: Must be able to stand, walk, bend, and lift up to 50 pounds with or without reasonable accommodation while providing direct patient care. Ability to safely assist with patient transfers, perform repetitive clinical tasks, and respond quickly in emergency situations is required.
